Continuity of Care
What happens after the outreach packs up.
What it is
Continuity of Care is a standing service rather than an event. It covers home follow-up for children needing wound care or treatment for common childhood illness, medical posts kept running during Academy events, and referral tracking from the day a case is identified to the day it is resolved.
Identifying a case takes a morning. Following it through to recovery takes months, and it is the part that decides whether an outreach mattered.
Who it is for
Free, and open to everyone in the community.
Anyone referred or identified through a MedWHOLE programme. Free, and open to everyone in the community.
Events
- 01Surgical follow-up at Gibonna Women's Hospital, GarkiQ2 2026. All seven cases identified at the 14 February Kunyami outreach underwent surgery and all seven recovered without complications.
- 02Teacher referral, TogeJune 2026. A 49-year-old teacher found with moderately elevated blood pressure was counselled and referred to the Primary Health Centre in Kuje, where she lives.
